If your state of residence (the state listed on your driver's license) is NOT on the list below, YOU ARE NOT ELIGIBLE FOR A WORKERS COMP ADVANCE AND YOU SHOULD NOT SUBMIT AN APPLICATION.

Applications received from Workers Comp claimants who do not live in one of the states below will not be processed and will be discarded without further notice.
